ENGLISH INSTRUCTIONS:
Please click on the applicable legal services agreement below. Each adult member (18+) of your household must sign their own, separate agreement. For any minor children, only 1 form needs to be completed by a parent or guardian. Please note that your enrollment will be confirmed only after you return the signed agreement as instructed.
Review all information thoroughly.
Fill out and sign the specified sections.
Press Submit once you have completed the agreement.
Repeat Steps 1-4 for each member (18+) by refreshing the page or re-clicking the link below.
